Notice:
A new Application Form for Refundable Accommodation Deposit (RAD) Approval will be available on the IHACPA website from 1 November 2025. If you submit an application on or after 1 November 2025, you must use the new form.
The new form will include changes to align with the Aged Care Act 2024 and the Aged Care Rules 2025.
Use this form to apply for approval to charge a RAD (or equivalent daily amount) that is more than the current maximum accommodation payment amount. The maximum accommodation payment amount is currently $758,627.

Supplementary form for additional accommodation groups
Use the main application form to record the information for accommodation groups 1 – 6. Use this form only to record information for accommodation group number 7 and onwards. See the supplementary form.
